Record Description : This series includes individual disease reports received by the Ohio Department of Health's Bureau of Infectious Diseases under Ohio Administrative Code 3701-3-02(A) and 3701-3-02(B). Information included in reports is described in OAC 3701-3-03 and 3701-3-04. Reports are entered into the electronic Ohio Disease Reporting System (ODRS) for appropriate public health surveillance follow-up. Reports of disease that are not specified in 3701-3-02 (e.g., "non-reportable" conditions) should be destroyed upon review, unless the file is covered under a separate schedule.
