Record Description :  Surface mine permit is that permit issued on or after July 1, 1975 for the mining of industrial minerals (IM) (other than coal) pursuant to R.C. Chapter 1514, but including incidental coal. This file contains records generally associated with the final approval/disapproval of each surface mine permit issued and those documents reflecting the final status of the IM site. Records in this file include, but are not limited to: approved application and permit, amendments, modifications, transfers and renewals of permit, lime mining waste disposal records, inspection reports; final bond release retiring permit or bond forfeiture, unabated enforcement actions, unpaid civil penalty assessments and their associated documentation, annual and final operator complaints, lease/deed affidavits, consents, geological data reports, hydrologic data, groundwater modeling 10'' regulatory cone of depression map, application map, cross section map(s), and final map. Records are added throughout the life of the permit.
