Record Description :  Surface mine permit is that permit issued on or after July 1, 1975 for the mining of industrial minerals (IM) (other than coal) pursuant to R.C. 1514, but incidental to coal. This file contains the records generally associated with the review of IM applications and field operations of each IM mine. Records include, but are not limited to: enforcement actions, including terminated Chief''s Orders, quarterly production reports, paid civil penalty assessments and their associated documentation, bond releases (except the final bond release retiring permit), bond receipts, bond invoices, photos (including slides, photographs and digital photos), modification and amendment maps, annual maps, groundwater modeling withdrawn applications, and production reports for incidental operations. Records are added throughout the life of the permit.
