Record Description :  Backgrounds (Criminal and/or Record Checks) are conducted for various State of Ohio agencies and boards & commissions, and for contractors/consultants contracted to work at ODPS facilities throughout Ohio, and for individuals wishing to enlist in a branch of the U.S. military. This record series may include, but is not limited to, the following: the request for criminal and/or record checks; applicant consent/release forms; supporting documentation presented at application; DD369 Police Record Check forms from U.S. military recruiting stations; criminal records obtained from law enforcement agencies and various courts; State (BCI) and/or Nationwide (FBI) criminal records; applicant’s fingerprints; driving records; data and documents collected during field interviews; background and polygraph result summaries; medical/psychological screening results; and letter/memo of approval or denial. Upon completion of the background, results are transmitted to the Requester, which shall become the official record. A copy of the record is retained in accordance with this schedule for the purpose of receipt confirmation and auditing purposes. NOTES: Backgrounds for employment at ODPS are retained in accordance with “Backgrounds and background checks for Employment” SAN 760-0788 (for hired applicants), “Application for Employment Bid Folders (Applicant Jackets)” SAN 760-1492 (for rejected applicants), or “Contract for goods, services, and property leases” SAN 760-1483 for contractors/vendors.
