Record Description : This record series contains the forms utilized by the Ohio Department of Public Safety (ODPS), Bureau of Motor Vehicles to provide Commercial Driver License (CDL) instruction and certification through a Third Party administrator. FORMS: BMV 5811 - "Application for Certification of Commercial Driver License Third Party Tester" - This application allows the prospective Third Party to apply for a position as a CDL Test site. The CDL Tester must maintain this record for 2 years after closure of CDL test site. BMV 5812 - "Application for Certification of Commercial Driver License Third Party Examiner" - This application allows the prospective Third Party Tester to apply for a position as a CDL test examiner. The CDL tester must maintain this record for 2 years after the conclusion of the CDL testing cycle. BMV 5813 - "Agreement for the Administration of CDL Skills Test" - This is a contract agreement between ODPS and a Third Party (the "Contractor") to administer CDL skills tests in accordance with the provisions of Ohio Revised Code (ORC), Chapter 4506, as the ORC may be enacted or amended from time to time, or instructions provided through the ODPS, and regulations of the Federal Motor Carrier Safety Administration. BMV 4891 - "Application for Class-D Third Party testing site (Pilot)" - This application allows the prospective Third Party Tester to apply for a position as a Third Party Class D test site as allowed in ORC 4507.112. The Third Party test site must maintain this record for 2 years after the conclusion of the Class D testing cycle.
