Records may include:, health care medical, dental and vision information, enrollment/change forms; COBRA eligibility notification; life insurance enrollment and beneficiary designation/change forms; annual affidavits of student and dependent status; Ohio Deferred Compensation forms; supplemental retirement account election forms; FBMC – Flexible Spending Account forms; leave conversion, donation and restoration forms; payroll deduction authorizations used to authorize deductions for charitable organizations, credit unions, US Savings Bonds, health insurance, or direct deposit; request for transfer of public service credit; Employers Identification Notice; state withholding exemption; out of state travel authorizations; taxpayer ID number and certification (W-9); statement of local tax liability; change of address; withholding allowance (W-4); employee suggestions; education volunteer request; telecommuting application; and, any additional supporting document relating to payroll and benefits.
