Record Description :  This record series contains accounts of individual customers (lessors or owners of apportionable vehicles), annual registrations and accounts of permit agencies and agents authorized to process and issue temporary credentials (72-hour Trip Permits) under the International Registration Plan (IRP), notifications for the registrations/license plates of all commercial motor vehicles assigned to a registrant or carrier with a shared USDOT#, and this record series includes: initial/renewal applications (OH-F5, Ohio IRP Application; BMV 4200, Permit Agency Application for 72-hour Trip Permits; BMV 4867, Notifcation of Revocation of Commercial Motor Vehicle Registrations/License Plates) and supporting documentation; registration fee invoices; trip permit agreements; trip permit agency weekly sales reports (BMV 4201); records of sold or voided trip permits; and miscellaneous correspondence. These records are subject to state and federal audits.
