Record Description : Releases of Information are: 1) requests for public information (not records); 2) requests for non-public information that are accompanied by authorization/consent forms signed by the person whose information is being released; and 3) requests for existing records that are accompanied by authorization/consent forms signed by the person whose record is being released.
This record series contains initial requests from the public, the Department''s letter of response, all correspondence between requester and the Department during the fulfillment of the request (if any), and may also include authorization/consent forms and imaged copies of records released in response to a person''s authorization/consent. Transaction histories for these requests are not entered or tracked in the Department''s Public Records Database.
