Record Description : Statement of benefits paid to combined wage claimants
which is sent to participating states on a quarterly basis. Each statement contains a list
of claimants whose out of state wages have been used to establish Ohio eligibility. Each
statement indicates the amounts of benefits Ohio is charging the participating state per
quarter for its participation of Ohio combined wage claims, and itemizes the amount
Ohio charges for each claimant listed on the statement. A “ledger record” is created in
FoxPro for tracking purposes and to balance receipts against disbursements.
