The list certifies that the Unemployment Compensation Review Commission decisions were enclosed in envelopes bearing the interested parties names and addresses and mailed on a specific date and time and processed accordingly by Ohio Department of Job and Family Services mail staff. Includes, but not limited to a daily listing of appeal docket number, clients name, employers, and addresses, total number of cases, and total number of actual mailings, signature of commission representative and mail staff representative.
