Record Description : This file consist of the doctors evalution of findings report and a brief description of the problem which has caused the deptartment to schedule the employee fitness evaluation. the file contains but is not limited to the employees fitness for duty, physical information,psychological information, advise as to whether or not the employee should recieve a copy of the report, if the employee is a threat to himself or others,disability application, letter to noitify employee of examination date, return to work letter,employee history screen, any correspondence between management used to determine if an evaluation is necessary, letter to notify employee of a meeting to discuss disability separation.
