Record Description :  File contains original registration, renewal, and transfer applications for Private Investigators and Security Guards. Registration - Under Ohio Revised Code 4749.06, providers must register each employee using form HLS0012. Three types of registrations are completed: new hire/rehire, name change, and card replacement. Under Ohio Revised Code 4749.10, providers must register each employee that intends to carry a weapon (FAB) using form HLS0069. Two types of FAB registrations are completed: intial firearm bearer and requalification firearm bearer. Included with this application will be any necessary paperwork, such as, but not limited to: BCI/FBI results, FAB certification, correspondence, and refund/invoices. Renewal - Under Ohio Revised Code 4501:5-1-20, providers must renew employee registrations annually using form HLS0027. Included with this application will be any necessary paperwork, such as, but not limited to: any correspondence and refund/invoives. Transfer - Under Ohio Revised Code 4749.06, providers must register each employee when they change branches using form HLS0013. Included with this application will be any necessary paperwork, such as, but not limited to: any correspondence and refund/invoices. These records were under jurisdiction of the Department of Commerce until July 1, 2004 (SAN 18000710)
