Record Description :  Under Ohio Revised Code 4749.06, Private Investigator/Security Guard providers are required to be licensed. These file will include the original licensing application (HLS0004) and all required licensing documentation, quarterly reports (HLS0011), annual renewals, proof of insurance, termination reports (HLS0010), and miscellaneous paperwork. Additionally, any subsequent new/replacement qualifying agent applications (HLS0004), name/address change requests (HLS0008), license replacement requests (HLS0008), and requests for online access/updates. Branch offices will have a separate file. These records were under jurisdiction of the Department of Commerce until July 1, 2004 (SAN 18000710).
