Record Description : Registrations are issued for 100% service connected disabled veterans (DV) and/or former prisoner of war (POW), and medal of honor recipients with VA or military documentation. The former POW plate is available to the surviving spouse with an application and a certified copy of the death certificate. The registration is generated from the application with the title and military proof when new, transfer, replacement and corrections are issued on the following vehicles: Non-commercial trucks, passenger vehicles, motor homes, motorcycles, boat trailers, etc. The registration contains the license plate number, owner name, address, vehicle year, make, type, serial number, title number, odometer reading, purchase date, issuance date, expiration date, tax district, county code, previous application number, new application number, deputy name, agency number and location. Distribution: Original copy - Filed by form number in Gratis Unit, 1st carbon copy - Mailed to applicant with plate, 2nd carbon copy - Shredded in Gratis Unit.
