Record Description :  In cases where disciplinary action is contemplated and the affected employee elects to participate in the Employee Assistance Program (EAP), the disciplinary action may be delayed until completion of the program. An employee may volunteer to participate in the EAP program without any discipline being imposed. If an employee goes through HRM for assistance, the OEAP participation agreement packet is completed by the employee and the employer, and forwarded to EAP for his/her treatment. The packet is NOT retained in the employee personnel file, but kept in an HRM confidential file.
