Record Description : The inventory form is completed by the Special Plates Division and lists all chargeable and accountable items (license plates, validation stickers and handicap placards) being returned to the BMV Distribution Center for disposition due to obsolete, mixed, defective, error, damage or number duplication. The inventory sheet contains the item type, item code, starting and ending series of what was returned, reason, date, total and the signatures of the releasing and receiving divisions. Distribution: Original - Deputy Registrar Services Division, Audit Section; Duplicate - Deputy Registrar Services Division; Triplicate - BMV Distribution Center; Quadruplicate - Special Plates Division; Quintuplicate - Destroyed - no value; Sextuplicate - Destroyed - no value
