Record Description : A Change and Delete Form is completed by the License Support Services Section - Record Clearance Unit for changing information in the data system on an individual''s operator license field, such as:Driver license number, weight, height, sex, issuance date, expiration date, date of birth and social security number. The form can also be used to make changes to the actual driving record. If a change is requested by a court, law enforcement agency, individual or another BMV section, they complete the form and forward it to the License Support Services Section - Record Clearance Unit for key entry into the system. The form is assigned a key number and then stored by run date.
