Record Description : The ''Recap of Daily Telephone Activity'' form is completed daily by the Communication Section for the purpose of informing the Deputy Registrar Services Division, Administration Section on the total of incoming and outgoing telephone calls handled by the employees in the Communication Section. The daily report lists the date; employees'' names; source of call (deputy registrar, field personnel, Univac Telephone Company, public, car dealer, and/or Bureau); total call per each employee; total calls per day and any remarks. Distribution: Original - Deputy Registrar Services Division, Deputy Administrator; Duplicate - Deputy Registrar Services Division, Division Chief; Triplicate - Deputy Registrar Services Division, Communication Section.
