Record Description : In the event that a Deputy Registrar discovers any chargeable and/or accountable items or BMV equipment stolen from their place of business, a ''Stolen Items Affidavit''is completed by the BMV field representative. The affidavit contains the Deputy Registrar''s name, address, agency number, date, county, name of local law enforcement department contacted, officer''s name, a list of stolen item(s), circumstances, date that the BMV field representative was contacted, and signatures of the Deputy Registrar and the field representative. The affidavit, along with attached police report, is submitted to Central Operations(CO). Upon receipt of the affidavit, then information is keyed into the system . Distribution: Original - CO; Xerox copy - BMV Field Representative; Xerox copy - Deputy Registrar.
