Record Description : The inventory forms are completed by the BMV's field representatives and lists all chargeable items returned from a Deputy Registrar to the BWV Distibution Center for disposition due to errors, damages of number duplications. This form is also used when Deputy Registrars are checked out at the end of a registration year for reporting all chargeable items being returned for credit or debt to their accounts. The inventory sheet contains the Deputy Registrar's name and address, agency number, town; item code; starting and ending series of what was returned and reason; report/date; total; and the signature of the field representative, releasing Deputy Registrar, and and employee of the BMV Distribution Center. Distribution: Orignal - DRASS/ACSU(For Posting Purposes); Duplicate - DRASS/ACSU Triplicate - Bureau of Motor Vehicles, Distribution Center (Used for a backup document for salvage material); Quadruplicate - Deputy Registrar's Copy; Quintuplicate - Field Representative's Copy.
