Record Description : Individuals seeking initial certification as a First Responder, EMT-Basic; reinstatement of a certificate as a First Responder, EMT-Basic, EMT-Intermediate, or EMT-Paramedic; challenging an exam in lieu of doing continuing education for any level; and individuals seeking instructor certification must pass an examination approved by the State Board of EMS. These exams are administered by proctors hired by the Division of EMS. Individuals may register for an examination on line or my submitting a hard copy application. The application contains the individuals name, address, date of birth, social security number, type of exam requested, training institution, date and location of requested exam. Exam candidates are converted to exam rosters by exam date and location.
