Record Description :  In accordance with ORC 4765.11 the State Board of Emergency Medical Servics (via Division of EMS) issues a certificate of approval to institutions who meet the criteria outlined in the OAC to providing continuing education training for first responders and EMTS. These applications include documentation of the how and where the courses will be conducted. A certificate of approval is valid for a period of three years, at which time an application for renewal is submitted. Limited information from application (institution name, address, telephone number, fax number, contact person, as well as expiration date when issued, is maintained in an electronic data base.
