Record Description : Investigative files of the Youth and Family Ombudsmen Office related to complaints and general inquiries received, reviewed, and resolved by the office, stored in digital case management and record system. Confidential investigative records may include correspondence related to investigation, evidence (pictures, documents, video and audio recordings), supporting documentation, personal identification information, documentation of investigative activities, investigative summaries, investigative notes, legal consultations, and investigative staffing notes. Investigative files may contain copies of and/or references to confidential children services case information. Records and materials received in other formats are digitized (if needed) and uploaded to the case management and record system. Records are confidential and retained permanently.
