Record Description : Records received from various state and local agencies for contact tracing of COVID-19 including, but not limited to, questionaires, answers to the questionnaires, and lists of employees. These records may be completed by Ohio Department of Public Safety (ODPS) staff while conducting contact tracing. Furthermore, additional records may be created in connection or response to the information received. Such records may include confidential personal information and protected health information as defined in R.C. 3701.17(A)(2)(a).
