Record Description : Claim Files consist of an OUF-6 Unclaimed Funds Claim Form containing owner detail information provided by the original holder that reported and remitted the funds to the Director. The OUF-6 Form is completed and signed by the claimaint and submitted with documentary proof of ownership, including personal identification and/or tax identification number(s). Claim files also include correspondence exchanged between the Division and claimant as well as other documents relevant to the claim.
