Record Description : Pursuant to ORC 4765.06 Emergency Medical Services (EMS) organizations are required to submit information regarding the delivery of emergency medical services in this state. Data and information that is required to be submitted include name and address of organization; time and date of incident; location of incident; EMS personnel involved; patient identification; information regarding provider assessment; description of treatment provided; and transport information. Data is submitted electronically and maintained electronically. Reports are generated from aggregate data.
