Record Description : Record series includes forms submitted to the Department by business taxpayers to request
to change filing frequency, request to file separately, request to change address, request to change election status, request to cancel or reactivate account, or request to file by paper. Series includes such administrative requests for Commercial Activity Tax, Petroleum Activity Tax, Financial Institution Tax, Sales/Use Tax, 911 charge, or resort tax. Series also includes any supporting documentation provided by the taxpayer to support their request.
