Record Description : File contains documentation of personal license issued by the State Cosmetology and Barber Board, including: examination application, examination results, work permit application, new license application, renewal information, correspondence to and from licensee/applicant and/or address change or additional license applications, education records and information, and/or reschedule applications. Series also includes inactive/lapsed barber license files containing proof of licensure of inactive/lapsed barbers, documentation of violations and discipline issued, and reinstatement actions taken. Series also contains records transferred on 01/21/2018 previously under series authorization number 50040007, agency series BRB-877002. As of 01/21/2018 the State Cosmetology and Barber Board processes and retains all records from the Ohio State Barber Board per the enactment of H.B. 49.
