Record Description : This form is submitted to us daily by Treasurer or State. The invoice is for fuson. decal, and meter impression sales to licensed wholesale cigarette dealers. An invoice is issued each time a wholesaler purchases indicia and includes the wholesalers name and address, invoice number, meter number, quantity purchased, gross dollar amount, discount, and net amount paid.
