Record Description : This report contains requests from individuals with diabetes whom want to become a school bus driver. Before consent from ODE, the following medical documentation must be submitted and approved: vital statistics, copy of valid commercial driver license, 3 year driving history (abstract), completed t-8 (3 pages), documentation and approval from Endocrinologist on driver history, medical history, documentation and approval by an Opthamologist, and either the approval or disapproval from ODE. Follow-up documentation for 3 years by Opthamologist and Endocrinologist @ 6 months intervals.
