CERTIFICATION DOCUMENTS AND AGREEMENTS: Retain for the duration of the agreement or certification plus two years, then destroy. ANNUAL REPORTS: Retain for five (5) years. Annual reports compiled by the Agency must then be reviewed by the State Archives prior to disposal. SUPPLEMENTAL ITEMS: Retain for five (5) years, then destroy. Certification of an area or program is based upon whether it has the characteristics set forth in the applicable section of the Ohio Revised Code.
