Record Description : Participant vehicle modification case files contain all records received and created in relation to the participant along with case notes that document progression of the vehicle modification case and all actions taken on behalf of the participant. The official records are maintained electronically in the case management system. In accordance with policy, all paper records are scanned and uploaded into the electronic case record within thirty (30) days of receipt and then may be destroyed. Records may include: Application, Certificates of Eligibility, Documentation of SD/MSD, Time Extension for Eligibility, Comprehensive Assessment, Individual Plan for Employment (IPE), Time Extension for IPE, Annual Review, closure forms, school and medical records, assessment, reports for community rehabilitation providers, surveys, correspondence, and any other received or generated similarly.
