Record Description :  Participant case files contain all records received and created in relation to the participant along with case notes that document progression of the case and all actions taken on behalf of the participant. The official records are maintained electronically in the case management system. In accordance with policy, all paper records are scanned and uploaded into the electronic case record within thirty (30) days of receipt and then may be destroyed. Records may include: Application , Certificates of Eligibility, Documentation of SD/MSD, Time Extension for Eligibility, Comprehensive Assessment, Individual Plan for Employment (IPE), Time Extension for IPE, Annual Review, closure forms, school and medical records, assessment, reports from community rehabilitation providers, surveys, correspondence, and any other record received or generated similarly.

Case files are maintained in an active or closed status. Active case files are retained until closure of the case. Closed case files are maintained for ten (10) federal fiscal years and then destroyed. A Certificate of Disposal is required when destroying these records.
