Record Description : The Move, Add, Change (MAC) and Equipment request form (DPS 0200) is used to provide supervisors and MAC administrators with procedures and checklists for account creation, access to data systems, equipment, and workspace set-up. This record series may include, but is not limited to, the following forms used to request computer hardware and/or software, remote access, portable computing device(s), cell phone(s), smartphones(s), and air card(s): DPS 0058 - Application for Remote Access; DPS 0060 - Data Services Request; DPS 0077 - Wireless Communications Request; DPS 0200 - MAC and Equipment Request; and DPS 0222 - Request for Portable Computing Device.
