Record Description : This series contains records of employment for each ODPS employee including: documents related to hire (application/resume and supporting documents, e.g., educational transcripts/diplomas, military discharge, character/professional references, and memo of approval); employee work rule agreements and acknowledgements; personnel forms, which may include, but are not limited to, ID badge information, requests for specialty positions, applications/approvals of secondary employment, Fair Labor Standards Act (FLSA) and essential employee designations, changes of address, and PERS form A; completion of service letters; personnel actions (not including disciplinary actions with the exception of termination); letters of commendation and recognition; emergency contact information; and exit interviews.
Note: The following personnel records are retained separately: Backgrounds (background checks) and polygraphs conducted for employment in accordance with SAN 760-0788 and SAN 760-0773; Employee medical and benefits records in accordance with SAN 760-1443 (non-OSHP/OIU employees) and SAN 760-1072 (OSHP/OIU employees); Employee training records in accordance with SAN 760-1538; Employee performance evaluations in accordance with SAN 760-1437; I-9 Employee Verification Forms in accordance with SAN 760-1532; and Employee disciplinary records in accordance with SAN 760-0787.
