Record Description :  Record of potential fathers filed by mothers in adoption cases to search the system to see if the father is registered. Files may include the Registration form, JFS-01694 (Father’s form that is filed if he wants to prevent the adoption), letter of receipt, Search Request form, JFS-01695 (Mother’s form that is filed to look for a system match), and the Preliminary Certification Notice, JFS-01696, which contains names, birth dates, SS#’s, and addresses for the mother, father and child.
