Record Description : Records include receipted license application forms, licensees' compliance statement and related documentation which serves as evidence of compliance with statutory requirements, reports of facility inspections by building, fire and health departments, inspection reports and related investigatory summaries of the centers. Files may also include copies of medical statements, material mandated documents of administrative hearings and Common Pleas Court proceedings, alledged operations, daycamps and additional license documentation. Step Up To Quality records may contain application, Star Rating Certificate, Onsite Inspections and visits, correction action and compliance, deferral and correspondance documents, appeals, supporting documents and additional license documentation. Type B Home License reports may contain electronic and paper Type B Home applications and county reccomendations for licensure, annual inspection documents including but not limited to: home inspections, records reviews, and background check reviews, corrective action plans, complaints, appeals and revocation.
