Record Description : The application is a legally binding agreement between the customer and the Ohio Tuition Trust Authority (OTTA). The application allows a customer to open a 529 account with the Ohio Tuition Trust Authority. It contains personal information for an account owner, beneficiary and successor. The original fund choice and contribution type is designated with the application. Future allocations are optional, but could be included on the application. It is considered a confidential record according to Ohio Revised Code 3334.11.
