Record Description : This series consists of records pertaining to commercial driver license holders'' self-certification forms and medical examiner certificates (MECERTs) in accordance with ORC 4506.10 and 49 CFR 383.73(o)(1)(ii). Records retained in this series include MECERT cards, self-certification forms (BMV 2159), and supporting documentation; MECERT notices of update, status change, or expiration; MECERT notices and case documentation supporting the downgrade, suspension, or revocation of CDL privileges based on MECERT violations; and MECERT transaction histories.
