Record Description : This record is a quarterly production report which is based upon data of Ohio employers and sent to the Department of Labor. It is referred to as the 581 Report. Information collected from the Tax Sections include, but is not limited to, total receivables for employers, timely reporting by employers, new business established and audit activity. The Bureau of UC Tax furnishes the data that the goes into the original report forwarded to LMI who sends it to DOL. Copies of the report go to all UC Tax sections.
