Record Description : Form GO2712 is used as a cover sheet for assignments
completed by field audit staff. The form lists, but is not limited to, history of work on an
employer account, type of assignment, contribution tax report, benefit payment
information, audit findings, inquiries and collections. All documentation pertaining to the
completed assignment is attached to the cover sheet. The name of staff person handling
the assignment and the date the assignment was completed is also listed.
