Record Description : This record series is retained at BMV Columbus and consists of the following information with regards to every deputy registrar within each district: Contract, contingency list, surety bond, information sheet for deputy registrar appointment, deputy registrar schedule, proof of business insurance, and Deputy Registrar Request for Proposal; Backgrounds completed by the Bureau of Criminal Identification and Investigation for deputy registrar employees; Deputy registrar political contribution statements; All records that deputy registrars maintain in accordance with section D of the Deputy Registrar Manual to include end of day reports, consignment invoices, Missing Affidavits (BMV5702), chargeable and non-chargeable inventories, Error Correction Worksheets (BMV5716), and deputy registrar annual inventory reports; Financial reviews, field representative files, lease information, training records, and digital site photographs; and correspondence, broadcasts, evaluations, work schedules, contracts, motor voter transmission forms, and abstract files, etc.
