Record Description : This form (ADM4107) is completed by division and submitted to HRM for upgrading a current position, creating a new position, or updating the position description. The form contains agency number, division, unit/office, position control number, title of position, title of immediate supervisor, class number, class title, county of employment, job duties required for that position, and the minimum acceptable requirements. Through decntralization, the agency now approves the new and updated forms. The original is kept in a separate file. A copy of the form is forwarded to DAS for their records.
