Record Description :  The Medical Provider Complaint Files contain complaints and correspondence concerning the medical providers. The Medical Provider Complaint Files are scanned into the Universal Document Service (UDS). The Universal Document Service (UDS) is an imaging concept that stores any type of electronic file, such as Microsoft Office documents, Adobe Acrobat, audio/video, Computer Aided Design (CAD), etc. as well as scanned images. The Disability Evaluators Panel (DEP) recruit, credential, and maintain the DEP physician network that provide independent medical exams and medical file reviews for BWC statewide. The DEP provides resolutions on all DEP inquiries including billing and exam complaints.

The Medical Provider Complaint Files are located on the DEP SharePoint site/DEP Complaint Tracker in an Excel spreadsheet. Retain until the provider is terminated from Disability Evaluators Panel (DEP) plus an additional 7 years. The Medical Provider Complaint Files are scanned and maintained in the UDS Doctor File. Retain the Provider Complaint Files in UDS until the provider is terminated from the Disability Evaluators Panel (DEP) plus an additional 7 years. The machine readable information will be reviewed every 3 years to ensure the Medical Provider Complaint Files can still be accessed. Need the information because the provider may apply for reinstatement to the Disability Evaluators Panel (DEP) and for possible litigation. Electronic Delete
Digitally image the Medical Provider Complaint Files. After the scanned versions of the Medical Provider Complaint Files have been verified for quality assurance, send to Mafil, and then destroy. Paper Shred
