Record Description : Under Section 4503.10 of the Ohio Revised Code, any owner of a motor vehicle who operates on the streets and highways shall file each year with a deputy registrar of the Bureau of Motor Vehicles an application for vehicle registration (BMV4603, BMV4616, or BMV4617). The computer-generated form is signed and dated by the applicant for registration issuance by mail. The form contains the owners name and address; plate category; type of plate; whether a power of attorney form is required; EPA inspection required if residing in Butler, Clark, Clermont, Cuyahoga, Geauga, Greene, Hamilton, Lake, Lorain, Medina, Montgomery, Portage, Summit, or Warren counties; owners social security number or tax identification number; taxing district/name; purchase date; old application number; new license number; registration expiration date; renewal expiration date; vehicle type, year and make; VIN number; title number; amount paid; dontation (in dollars and cents amount) to Save Our Sight campaign; space to fill out any changes in applicants name or address; applicants signature and date. The applications are imaged according to issuance dates and batch control numbers. Power of attorney forms and EPA inspection certificates are also imaged. EPA inspection certificates are stored with the applications, and the power of attorney forms are stored in separate boxes using the same batch numbers as the applications they belong with. Under Senate Bill 1, in December 1988, the "Mail-In Registration" went into effect. Effective May 2002, all mail in applications are processed through the BMV Remittance Processing Unit. Individuals have a choice to register their motor vehicles by a deputy registrar, mail-in registration, using the OPLATES.com website, or the BMV automated telephone service.
