Record Description : Under the Drivers License Law sections 4507.08, 4507.081 and 4507.20 of the Ohio Revised Code, any person who is afflicted with or suffering from such physical or mental disability or disease and is sufficiently under medical control that the person is capable of exercising reasonable and ordinary control of a motor vehicle, may have a restricted drivers license issued. Records include copies of form BMV 2310 Request For Statement of Physician, Incomp Report, Documents from Courts, Counselors, Rehabilitation Centers, Eye Doctors, Low Vision Clinic, Exam Stations, OSU Clinic, various physicians and other Medical Assistants, Copies of statements from Law Enforcements and Courts.
