Record Description : To provide a uniform means of collecting and recording statewide Computer Aided Dispatch (CAD) Personnel Onformation. Personnel information captured on the form included an employees full name, personnel number, emergency contact information, blood type, medical alert or condition, and any special skills the employee may retain including being fluent in a foreign language. All information is entered in the CAD personnel database and can be accessed statewide at any dispatch console. Once up-to-date form for each OSHP employee should be kept on file with GHQ/TCS at all times.
