Record Description : Under section 4510.03 of the Ohio Revised Code, conviction tickets are received from the county court of record or mayors court resulting from violations of law or ordinance regulating the operation of a motor vehicle on the streets and highways. These are all major convictions (6 point traffic tickets, under sections 4507.16, 2903.06, 2903.07 and 4511.191 of the ORC, and any traffic tickets specified under sections 4549.021 and 4549.03 of the ORC. The tickets contain the violators name and address, drivers license number, license plate number, date, date of birth, name of the court, court case number, nature of offense, date of hearing, number of points charged, suspension date, fine, violators social security number, and the signature of the court clerk. In accordance with the provisions of section 4507.16 Division (B)(2) of the ORC, the suspension period shall be totaled on a five year basis from date of first conviction.
