Record Description : This series includes the drug analysis application submitted to the Ohio Dept. of Health''s Bureau of Alcohol and Drug Testing. Included on the application is: the applicant''s name, the name of the laboratory where they are employed, telephone number of the facility, position the applicant is applying for, name of national proficiency testing program, applicable education and degrees, the applications''s signature and date. The director of the laboratory, where the individual will be working, must relate that the laboratory has a current and an updated manual and internal policy which is posted in the area where the individual will be conducting the drug analysis work, in compliance with 3701-53-06 and he also must verify the quality of the lab, the qualifications and certification of the applicant and the director, and that the director will be responsible to monitor the work and quality of work completed by the applicant.
