Record Description : As defined under section 4503.44 of the Ohio Revised Code, any person who has lost the use of one or both legs or one or both arms, who is blind, deaf, or so severly handicapped as to be unable to move without the aid of crutches or a wheelchair, or whose mobility is restricted by a permanent cardovascular, pulmanary, or other handicapping condition may apply for handicap license plates. A form "Physicians Certification of Eligibility for Handicap License Plates" must be completed by the applicant and physician. This form is then submitted to the Special Plates Division along with the Registration Application. The form contains the applicants name and address, physicians signature, physicians name, physicians license number and address. NOTE: The Physician Certification is a one-time submission.
